Parental consent for a rolling programme of series & local visits

Due to the current Covid situation, we are trying to minimise sending paperwork home for completion. We are therefore trialling an alternative way of completing documents that are required to be sent annually to parents.
Each year we ask parents to give consent for their children to take part in local off site visits. These are short visits for example to the local park, village hall or church and would not require additional consent prior to the visit. All other visits would still require the usual consents prior to these going ahead.
Please could we ask all parents to click on the link below and complete the questions to give consent for local off site visits. Unfortunately, if we do not have consent for each child, they would not be able to take part in these off site visits.
