Rolling Consent Form, 2024-2025.
This consent enables your child to attend various events, e.g,
swimming, forest school, walking to church etc without the need to fill in a consent every time.
Siblings can be put on the same form if they have no medical/allergy or dietary issues. If they have, please fill in a separate form.
